TL;DR
For most tourists entering Georgia, travel insurance is usually not checked and is generally not a standard requirement for visa-free entry, but it is still strongly recommended. If your stay is open-ended, travelers usually buy a 30–90 day policy or cover their expected stay and extend it later if needed.
If you are doing a visa run and do not know your exact departure date, a practical option is to buy insurance for at least the first 30 days in Georgia. That is a common starter period, and many travelers extend the policy later if needed.
Based on traveler experience, insurance is usually not requested at the Georgian border for a standard entry through Tbilisi, but traveling without it is still risky. At a minimum, it makes sense to cover the part of the trip you have already planned, such as 30 or 90 days.
If you genuinely do not know how long you will stay, travelers usually choose a fixed policy period with some buffer rather than an open-ended plan—for example 90 days or even 1 year for a long flexible stay. For a short trip, though, an annual policy is often unnecessary.
