TL;DR
On the allindonesia site you must fill out a separate application for EACH traveler individually, not add everyone to one form — otherwise you'll get only one QR code for the whole family but be able to pay for just one visa.
The whole process is fully electronic: no payment receipt needed, only your flight ticket — the visa itself exists in the system linked to your passport.
Known system bug: if you fill one application for the whole family (checking 'no visa needed' and then paying for a visa at the end), you may receive only one QR code per submission even though payment is charged for everyone — and the visa check website shows no data. Be prepared to get the visa on arrival at the airport if this happens.
On the allindonesia site: after filling out the form and getting your main QR code, check the 'I need a visa' box — you'll be redirected to visa payment; once paid, both the visa and QR code arrive immediately, no photo upload required.
If traveling with multiple people, fill out a completely separate application and separate visa payment for EACH person, instead of adding family members into one form — otherwise you'll get a single QR code for the group but will only be able to pay for one visa.
